1. Introduction to ICD-10 Coding
This chapter will provide clinicians with information about the World Health Organization’s (WHO) International Classification of Disease (ICD), 10th edition, overview of requirements for code selection, and application in the home health post-acute setting. Importance of proper ICD10 coding in the home health post-acute setting will be discussed in relationship to regulatory requirements, payment and assumed behavioral adjustments from the industry, with the movement to the PDGM January 1, 2020.
2. Primary and Secondary Diagnosis Selection
This chapter will provide clinicians with recommendations, guidance and examples of both sufficient and insufficient documentation of care in the home health setting related to diagnosis coding in the PDGM. The role of admission and clinical evaluation in the process of accurate diagnosis coding will be outlined specific to conditions commonly treated in the home health setting.
