Hospice HOPE Tool Explained
A big change is coming for hospice providers. On October 1, 2025, the new patient assessment tool from CMS, Hospice Outcomes and Patient Evaluation, or “HOPE,” will go into effect. This new tool utilizes assessment-based quality data to enhance the Hospice Quality Reporting Program (HQRP). Now is the time for hospice clinicians to learn how to use the HOPE tool and prepare for how it will change their practice.
In this recorded webinar, Jennifer Kennedy, VP for Quality and Standards at Community Health Accreditation Partner (CHAP), will discuss the new regulatory changes coming with the HOPE assessment tool and how it will contribute to the patient’s plan of care. She’s joined by Anna Zimmerman, MSW, Sr. Program Manager, Home Health & Hospice, to explore opportunities for hospice clinicians to adjust their practice and prepare for the new regulations.
